The validity of the registration and consent statements of users and other individuals under guardianship who have not reached the age of 16 requires a consent statement from their legal representative (usually the parent). The consent statement can be downloaded from the link below. For online registration, the form completed by the legal representative, available for download on the website, must be submitted to the Data Controller in person, by email, or by post during the minor’s first visit within 30 days. Otherwise, the conditional registration will be deleted.
